TERMS OF PARTICIPATION

I, as parent/guardian submitting this form, give my permission for my child, to attend and participate in this activity sponsored by Strong Rock Christian School. Strong Rock Christian School rules, regulations and dress code will be in effect on this field trip.

The undersigned authorizes an adult, in whose care the minor has been entrusted, to consent to any X-ray examination, anesthetic, medical, surgical or dental diagnosis or treatment and hospital care, to be rendered to the minor under the general or special supervision and on the advice of any licensed physician or dentist.

The undersigned is liable and agrees to pay all costs and expenses incurred in connection with such medical and dental  services. The undersigned does also give permission for my child to ride in any vehicle designated by the adult in whose care the minor has been entrusted while attending and participating in the activities sponsored by Strong Rock Christian School. In no way shall the undersigned hold Strong Rock Christian School and its representatives accountable for any injury and/or subsequent expense incurred by the participant.

By clicking agree below, the parent/guardian acknowledges and accepts the risks of physical injury associated with participation in the activities of Strong Rock Christian School. If a dispute over this agreement arises, the participant agrees to resolve the matter through a mutually acceptable arbitration process.

All staff and 5th-12th grade students will be issued ID Badges/Key Cards with their photos. The ID Badge/Key card will provide access to enter the campus buildings during the school day. The ID Badges/Key Card, including a lanyard and a plastic sleeve are provided free of charge. If a student loses their ID Badge/Key Card, a replacement card will cost $25.00. Students should always have their card with them, but it does not have to be around their neck. Although the students are receiving a Strong Rock lanyard, students may opt to wear their own lanyard; however, it must be Strong Rock appropriate. Younger students will continue to use ID Cards as they have in the past.

Information Regarding Your Child

I hereby give my permission for (child's name entered below) to take part in various sponsored trips, outings, camps, and events of Strong Rock Christian School. I further give my permission for the designated/approved school representative or sponsor to secure any needed medical treatment for the above named son/daughter. I release the school representative or sponsors from liability for accident or injuries on these trips or activities.

I further understand and agree that in the event that the above named son/daughter is involved in any inappropriate or dangerous activities, I will pay his or her expenses to be sent home immediately at the discretion of the school personnel.

Media Consent. I give my consent and permission for the taking of photographs and/or video of my child during Strong Rock Christian School sponsored events and waive and/or assign any and all rights (including copyright) in such media to Strong Rock Christian School.

Strong Rock Christian School, as the sole owner of such media, shall have the exclusive right to control and determine the use, display, performance, reproduction and dissemination of any such photographs and/or videos.

Time: 7 AM - 7 PM (approximately)
Where: Meet at SRCS, then to Camp Grace in Roberta, GA. We will return to SRCS before dinner and the home football game.

Plan: Join us for a day of teambuilding, adventure, food, and growing as an 8th grade class! (Students only)

Parking Passes:

No student may drive and possess a vehicle on campus until he/she has a valid Georgia driver’s license. Strong Rock Christian School is a closed campus; students are not permitted to leave the premises until dismissal time without special permission from school administration. Being able to drive and possess a vehicle on the premises of Strong Rock Christian School is a privilege and not a right; the privilege may be revoked at any time at the sole discretion of the school. Students who drive to school are required to purchase a parking pass. The cost is $75 for the school year and may be purchased in the academy/high school office.  Parking passes must be displayed in the car throughout the school year.
